Kindergarten Survey 2022-2023
Lincoln Elementary School District #27 would like to determine approximately how many kindergarten students to expect for the 2022-23 school year. Children must be five years old on or before September 1, 2022 to be eligible for kindergarten. If your child is eligible to enter kindergarten for the upcoming school year, please complete this Kindergarten Survey by March 12, 2022. If you would prefer, you can print out this form and return it to one of the school offices prior to March 12, 2022.
Information regarding Kindergarten Registration will then be e-mailed to you in April.
Before entering in the fall, we will need a copy of your child’s physical and immunization record, dental and eye exam, and a certified birth certificate.
